Job Title: Scheduler – Home Health & Home Care

Job Type: Full-Time
Reports To: Administrator for Home Care / Director of Professional Services for Home Health

Position Overview:
We are seeking an organized, detail-oriented Scheduler to join our growing home health and home care agency. This individual will be responsible for coordinating caregiver-to-client and clinician-to-patient schedules to ensure timely, quality care delivery. The Scheduler will serve as a vital link between our clients/patients and our field staff, supporting both clinical efficiency and client satisfaction.

Key Responsibilities:

  • Coordinate and schedule home care caregivers with appropriate patients based on location, availability, and care needs.
  • Match caregivers with home care clients, ensuring compatibility and continuity of care.
  • Maintain accurate and up-to-date scheduling records in the agency’s scheduling software and EMR.
  • Communicate schedule changes promptly to staff, clients/patients, and management.
  • Manage last-minute call-offs, cancellations, and rescheduling with efficiency and professionalism.
  • Collaborate with the clinical and administrative team to ensure staffing meets the patients’ plan of care and agency protocols.
  • Ensure compliance with state and federal regulations regarding staffing and service delivery.
    Serve as the first point of contact for scheduling inquiries from staff and clients.
  • Maintain confidentiality of client/patient and employee information.

Qualifications:

  • High school diploma or equivalent required; associate or bachelor’s degree preferred.
  • 1–2 years of experience in home health, home care, or medical scheduling strongly preferred.
  • Knowledge of home health regulations and scheduling software (e.g., Axxess, SmartCare, AlayaCare, or similar) is a plus.
  • Strong organizational skills with the ability to multitask in a fast-paced environment.
  • Excellent verbal and written communication skills.
  • Ability to build rapport with staff and clients and resolve conflicts with professionalism and empathy.
  • Proficient in Microsoft Office (Word, Excel, Outlook).

Work Environment:
This position is primarily office-based but may offer hybrid or remote scheduling flexibility depending on agency policy.

Schedule:
Monday to Friday, 8:30 AM – 4:30 PM, with occasional on-call or after-hours coverage as needed. (May vary depending on needs)
