🌟 Home Health Physical Therapist (PT) – Full-Time or Part-Time

Make a difference where it matters most — at home. We’re looking for a dedicated Physical Therapist (PT) to join our compassionate home health team. Help patients regain mobility, independence, and quality of life — all from the comfort of their own homes.

🔑 Key Responsibilities:

Provide skilled physical therapy services in patients’ homes according to the plan of care.
Perform evaluations, develop and revise treatment plans, and set measurable goals.
Educate patients and families on exercises and self-care to improve mobility and safety.
Conduct home safety assessments and recommend modifications as needed.
Collaborate closely with physicians, nurses, and other team members to ensure quality outcomes.
Maintain accurate, timely documentation for each visit.
Participate in case conferences and communicate patient progress to the care team.
Support the patient and family in understanding goals, expectations, and progress.

✅ Requirements:

Current Physical Therapist (PT) license in Nevada (required).
Minimum 1 year of home health physical therapy experience (preferred).
Strong clinical, communication, and problem-solving skills.
Ability to work independently and manage a flexible caseload.
Valid driver’s license and reliable transportation.

Compensation: $50.00 - $70.00 per hour
