Responsibilities:
- Conduct thorough assessments and provide quality healthcare services
- Collaborate with our interdisciplinary team for seamless patient care
- Ensure accurate documentation and timely follow-ups
- Facilitate smooth transitions for patients post-acute or skilled stays
Requirements:
- Current Nevada NP or APRN license
- Minimum two years of experience in primary care or geriatrics preferred
- Strong clinical skills and communication abilities
- Commitment to ongoing professional development and licensure requirements
- Positive attitude and dedication to patient-centered care
