Job Summary:
We are seeking an experienced and dedicated per diem Physical Therapist to join our dynamic home health team. The ideal candidate must have a strong background in home health care, demonstrating proficiency in assessing, planning, and implementing effective physical therapy interventions for patients in the comfort of their homes. As a per diem Physical Therapist, you will contribute to the overall well-being of our patients by promoting mobility, independence, and optimal functioning.

Responsibilities:
  • Conduct thorough assessments of patients' physical conditions, identifying areas of impairment, functional limitations, and setting realistic goals for rehabilitation.
  • Develop and implement individualized treatment plans based on assessment findings, incorporating evidence-based practices to maximize patient outcomes.
  • Provide skilled physical therapy services to patients in accordance with physician orders, addressing issues such as mobility, strength, balance, and pain management.
  • Collaborate with interdisciplinary team members, including nurses, social workers, and other healthcare professionals, to ensure comprehensive and coordinated care.
  • Educate patients and their families on therapeutic exercises, safety measures, and home exercise programs to enhance treatment outcomes.
  • Document patient progress, treatment interventions, and any changes in the patient's condition accurately and timely in accordance with regulatory standards.
  • Maintain open communication with the clinical supervisor, attending regular meetings and participating in case conferences to discuss patient care plans.
  • Stay current with advancements in physical therapy practices and home health regulations, incorporating new knowledge into clinical practice.
  • Adhere to ethical and professional standards, ensuring patient confidentiality and promoting a positive and compassionate care environment.
Qualifications:
  • Valid NV state license as a Physical Therapist.
  • Minimum of 1 years of experience in home health physical therapy.
  • Strong clinical skills with a focus on patient-centered care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and collaboratively within a team.
  • Understanding of Medicare home health regulations and documentation requirements.
  • Current CPR certification.
  • Reliable transportation for home visits.
